Bariatric pathway

Bariatric Nutrition Education

A comprehensive education pathway for patients preparing for weight-loss or revision surgery at St. Agnes Hospital, with individualized support before and after surgery.

  • One-hour consultation + 45-minute video
  • 4-hour long-term education class
  • 6-hour medical nutrition pre-op class
  • 2.5-hour post-surgery transition class
  • Tuesday Zoom Q&A access when eligible

Support: Before surgery, through early recovery, and for long-term needs

Education classes: $85–$175; consultation and support services priced separately

Healthy eating

ANRP

Achieve Nutrition Results Program is for people who have not had bariatric surgery and are not pursuing surgery. Choose only the components useful for your goals.

  • 5-hour Healthy Eating & Weight-Management Video Series
  • One-hour individual assessment
  • Personalized nutrition plan
  • 30- or 60-minute follow-ups
  • 15-minute phone coaching

Support: Flexible combination based on individual needs

Consultation: $125; 30-minute follow-up: $65; phone coaching: $20

7-week course

STRIVE BLE

A recorded behavioral-learning experience led by Nancy, a therapist, and participating patients, combining nutrition coaching with Cognitive Behavioral Therapy principles.

Bariatric Life Empowerment (BLE)

An extended bariatric behavioral program combining recorded learning with weekly reflection and live support from Nancy and a therapist.

Insurance documentation

Supervised Weight Documentation

Ongoing classes and documentation for bariatric patients whose insurance requires supervised nutrition and weight-management education.

  • Digital 24-hour dietary recall
  • Lifestyle assessment
  • One or two habit goals per session
  • Documentation sent to the surgical coordinator
  • Insurance-specific pathways when required

Support: Typically 3–6 months; insurance requirements vary

$40/month standard; Aetna $240 for 12 sessions; BC/BS coaching $45 total or $15 per call
